New Faculty: Orientation Checklist This checklist has been created to be used by new faculty members. The goal is to provide faculty members with standard issues or processes which should be reviewed with ____________________. (Department identify contact for new faculty member)
Prior to Start Date
Receive, complete and sign the following documents and return to the appropriate department designate.
Offer letter
Clinical Contract if applicable
Employment Application
Send to Credentialing Contact within Department or BJC’s Central Verification Office (996-3180) as defined by your hiring Department.
Missouri State Standardized Application form
Standardized Credentialing Form Supplement
Professional Reference List
Authorization & Release Form for CVO
Board Certification Certificates, if applicable
Current State Medical License
DEA Certificate
BNDD Certificate
Curriculum Vitae
Credentialing Information Form
For hospital privileges a separate credentialing process needs to be initiated through the hospital Medical Staff Office. This process requires additional documentation above and beyond what is completed for WUPN (Washington University Physician Network) If you have questions check with your department contact for this information or BJC’s Central Verification Office at 996-3180.
The following forms will be given to you with instructions on signatures needed; where to send; and by what date due:
The new faculty orientation program is offered annually. Communication is sent to departments and new faculty members with the details for registering. Information is also included on the Office of Faculty Affairs website.
For those with CLINICAL ROLES (forms are included with offer letter and department identifies who should receive documents.)
Effort Allocation Form – required to determine how effort will be allocated among the categories of clinical, research, administration, etc. (all faculty )
AAMC form (all faculty)
Expert Witness Testimony
Non-Compete Contract - Policy (Investigator & Clinician Track faculty need this policy)
The First Day in the Department
Complete any remaining new-hire paperwork.
Faculty who will have a clinical role:
Have photo in white lab coat (clinical faculty only)
Complete private consultation contact Info (pagers, phone, exchanges) for Referring Physicians Directory (clinical Faculty only)
Emergency Medicine Physician Preference Form (clinical Faculty only)
Obtain WUSM identification badge and information regarding parking from department contact.
Receive office/department keys from department contact according to appropriate safety and security requirements.

The First Week
Benefits/Insurance Enrollment – Enrollment must be completed within 31 days of hire date. To be scheduled to attend a benefits enrollment session. Call 362-7196.
Complete CME (Continuing Medical Education) Disclosure of Financial Relationship
For faculty who will be involved in RESEARCH
Obtain, review and retain the PI’s Quick Reference Guide to Managing Sponsored Funds from Research Education for those in research (Janice Knobloch 935-9789).
Provide resource materials, SOP manuals and other necessary tools.
